| |
| Intermediate Care |
| Residents requiring Intermediate Care Facility, (ICF) level of care do not qualify for Skilled Care under Medicare Part A but require assistance with 2 or more Activities of Daily Living (ADL), i.e., bathing, dressing, transfers, grooming, ambulation, toileting, and/or eating. |

| |
| Skilled Care |
| Residents requiring Skilled Nursing Facility (SNF) level of care meet Medicare requirements for Part A coverage and have a physician's order for care. These residents may need Physical, Occupational, or Speech Therapy 5 days a week. Part A Medicare coverage may last up to a maximum of 100 days, though it is rare for anyone to require Skilled Care for the maximum number of days. To remain on the SNF level of care, the individual must continue to need the skilled service and continue to be making measurable progress toward the rehabilitation goal as defined by Medicare. |

| Residential Care |
| Residents requiring Residential Care Facilit (RCF) level of care are residents who do not need as much assistance with their ADLs as the ICF resident. These residents may require assistance with bathing, laundry services, and meal preparation and service. |

| Outpatient Therapy Services |
|
The Sheffield Care Center offers outpatient therapy services through the Sheffield Rehab Center. The Center is located at 100 Bennett Drive and provides outpatient physical, occupational, speech, and lymphedema therapies. Individuals who have sustained an injury from work, sports, surgery, accidents, etc. with a physicians referral will be served at the facility. Home bound individuals may also be provided therapy services within their own home. | Meals-on-Wheels |
The Sheffield Care Center offers a Meals-on-Wheels program to the citizens of Sheffield. Nutritious meals are prepared at the Care Center and delivered daily Monday through Friday, except on holidays, to consumers in their own homes. For more information about our meals-on-wheels program, please contact the facility Administrator at 641-892-4691. Our Meals-on-Wheels program is also approved by the Department of Human Services under the elderly waiver program.

| Assisted Living |
| In 2009 our dream of adding on an assisted living facility became a reality. Deerfield Place, a 16 unit assisted living facility, opened in December 2009. Deerfield Place offers studio, 1 bedroom, 1 bedroom deluxe, and 2 bedroom apartments to individuals needing assistance with activities of daily living.
